In the event of the death of a member or dependent, or diagnosis of a critical illness (if applicable to your plan), special claim forms and other documentation will be required before the claim payment can be processed.
Tip!
In the case of a death claim, you will be required to provide GroupHEALTH the employee’s original signed Enrollment Card.
Please contact your Client Service Representative or Benefit Specialist to obtain claim forms and assistance regarding submission of any claims for:
- Life Insurance
- Optional Life Insurance
- Accident and Serious Illness (ASI)
- Accidental Death, Disease & Dismemberment (AD&D)
- Critical Illness
